Summary

This single-center observational study included 48 unique patients with complete core clinical data. Chest radiography was available in 47, CT in 19, and thoracic/pleural ultrasound in 48 patients. Parenchymal infiltrates or pneumonia were reported on radiography in 37/47 (78.7%). CT identified pleural effusion in 14/19 (73.7%), bilateral pneumonia in 10/19 (52.6%), and interstitial injury/edema in 9/19 (47.4%). Ultrasound reports mentioned pleural effusion in 21/48 (43.8%). For effusion, radiography yielded fewer positive reports than ultrasound (McNemar p=0.0026) and CT (p<0.001), with near-zero kappa values. No imaging phenotype showed a statistically confirmed association with PaO₂ or inflammatory markers after multiplicity adjustment. The modalities should be viewed as complementary. Prospective time-aligned imaging with standardized lung ultrasound scoring is required.
